You can either continue on this webpage to review the employee handbook and fill out the required forms. Otherwise, you can definitely  just get a paper copy of the employee handbook and required forms on the first day.

2. Below are the required forms that are needed before you first paycheck is issued. If you wish to fill them out prior to day 1, the W-4 and WT-4 forms can be filled in online & printed out to help make them easier to read. The I-9 and direct deposit form must be printed & filled in manually. 

First day:

When you fill out the required forms please hand them and also present a valid driver’s license or current form of picture identification and your social security card.

Required Forms :

1. Form W-4 for Federal Income Tax Withholding
Please print and submit page one only.

2. WT-4 Wisconsin Income Tax Withholding Form
Please print and submit page one only.

3. Form I-9 for Employment Eligibility
Please fill out page one, print out pages one and two only, and bring in the two required forms of identification as listed on page 3.

4. Direct Deposit Form
Mid Vallee uses Direct Deposit instead of paper paychecks. This allows you to have access to your pay as soon as it becomes available. Each new employee must complete this direct deposit form during his/her first week of employment. Please complete this form and return it with a voided check or deposit slip for each account. If you provide a printed form supplied by your financial institution, you do not have to attach a cancelled check or deposit slip.

OPTIONAL MIDVALLEE APPAREL: If you wish to order optional Mid Vallee Apparel, the online orders are due the 15th of either April, May or June. As a thank you to our team, we have also added a checkout discount of 25% off your FIRST order (we have youth sizes, tall/plus sizes, 60th apparel, tons of colors, etc)!  CLICK THE LINK HERE. Sample sizes are available, ask a shift manager. Orders are shipped to Mid Vallee 2-2 1/2 weeks after the due date of the 15th. Also, any merchandise in the proshop is offered at a 20% discount for employees (reach out to management for questions).
